New behavioral health integration coverage
The Connecticut Department of Social Services says the Connecticut Medical Assistance Program (CMAP) began covering and reimbursing the psychiatric Collaborative Care Model, or CoCM, for HUSKY Health A, B, C and D members.
According to the department’s Provider Bulletin 2026-36 and Policy Transmittal 2026-25, the policy applies to dates of service beginning July 1, 2026. The bulletin is addressed to outpatient hospitals, physicians, advanced practice registered nurses, certified nurse midwives, physician assistants, medical clinics, school-based health centers, behavioral health clinicians and federally qualified health centers.
What the model includes
The Connecticut Department of Social Services describes CoCM as a patient-centered, team-based approach for treating and managing behavioral health conditions in medical settings.
According to the bulletin, reimbursable CoCM services must be provided monthly. The required elements include use of an integrated patient registry, ongoing assessments with validated clinical tools, care management and patient engagement, patient-centered treatment goals, and tracking of individual progress and clinical outcomes.
The department states that all of these elements are required for a CoCM service to qualify for reimbursement. Providers are also directed to consult CoCM Frequently Asked Questions on the ctdssmap.com website for additional guidance.
Billing codes listed by the department
The Connecticut Department of Social Services lists three procedure codes for psychiatric Collaborative Care Model services. Code 99492 covers an initial service of 70 minutes per calendar month, according to the bulletin.
The bulletin lists code 99493 for subsequent services of 60 minutes per calendar month. It also lists code 99494 as an add-on code for an additional 30 minutes.
The department’s notice concerns coverage and reimbursement through CMAP for eligible HUSKY Health members. It does not describe CoCM as a replacement for other behavioral health services.
